Nurses Network, Northern Arizona’s longest standing, privately-owned home health agency is currently seeking dedicated and compassionate healthcare professionals to join our team of skilled home heath care providers. The ideal candidate will have a strong commitment to providing quality care to our patients in their homes.

POSITION SUMMARY :
The RN is experienced in planning, organizing, and directing home care services with an emphasis on community health education. The professional RN builds from resources to plan and direct services to meet the needs of individuals and their family members within their homes and communities.

• This is a Mon-Fri position, may consider PRN for the right candidate.
• Completes an initial assessment of patient and family to determine home care needs, including a complete physical assessment and history of current and previous illness(es).
• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan involving the patient and the family to establish goals based on nursing diagnosis incorporating therapeutic, preventative and rehabilitative nursing actions.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the Physician. Provides direct patient care as defined in the State Nurse Practice Act.
• Counsels the patient and family in meeting nursing and related needs.
• Provides health care instructions to the patient as appropriate per assessment and plan of care.
• Identifies discharge planning needs as part of the care of plan development and implements prior to discharge of the patient.
• Acts as Case Manager and assumes responsibility to coordinate patient care for assigned caseload.

QUALIFICATIONS

• Associate's or Bachelor’s degree with one year’s of home health care experience preferred.
• Must be licensed in the State of AZ as a Registered Nurse.
• Current BLS certification.
• Licensed driver with an automobile that is in good working order and insured in accordance with the state’s requirements.
• Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
• Visual acuity and hearing to perform required skills.
• Knowledge of EMR systems (Devero preferability)

Our services are prescribed by a physician to help medically qualified individuals with rehabilitation after an injury, hospital stay, surgery, or an illness.Our services may include wound care, IV therapy and PICC line maintenance, cardiac or orthopedic rehab, medication management, pain and symptom control, or help managing a chronic medical condition. We offer a wide variety of assignments for RN’s LPN’s and CNA’s in Northern and Central Arizona, including but not limited to: Assisted Living Facilities, Independent Living Facilities, Long-Term Care Facilities, Adult Day Care, In-Patient Hospice Settings, Clinics and Surgery Centers, Psych Rehab Centers, Detox Treatment Centers, and Rehabilitation Hospitals. Positions in all departments are open in all three of our Northern Arizona locations: The Prescott area, The Verde Valley and Flagstaff!
